The Mount Pleasant Town Clerk maintains a permanent record of all Births and Deaths that have occurred at Westchester Medical Center (formerly Grasslands Hospital) or within the Town, as well as Marriage Certificates for those who obtained their marriage license from the Town of Mount Pleasant, regardless of where the marriage ceremony occurred.
Through this portal, you may make an official request for a birth, death, or marriage record. These records are legal documents and should be requested only by someone who is legally entitled to the record.
Please complete the request in full and finalize the order process through payment of the 'order fee'. If you choose to pick up your document(s), you will be notified by email when your order is ready.
The fee will be collected by Permitium Payments and will show on your credit card statement as 'PermVitalRecs,' a secure and trusted provider of payment services.
